On October 15th & 16th, a group of delegates joined PRS founder Gary Tyne CRL, for our interactive, two-day Root Cause Analysis training course.
Designed by RCA experts, the two-day course, held in Huddersfield, outlined the practical applications of Root Cause Analysis, utilising 5 Whys and Cause and Effect.
From understanding the impact of a problem, delegates were guided through developing analysis and identifying the solutions to the problem, to arm them with the ability to help solve issues in their own work environments.

Adding value to any organisation, Root Cause Analysis brings together causes from various sources, linking them into a logical “big picture” relationship, supported by evidence. Identified solutions are made clear, and the investigation becomes part of corporate memory, helping to improve the bottom lines of the businesses utilising RCA.
Keen to provide an interactive course, packed with detail, with a good steady pace of learning, our delegates explored the whole Root Cause Analysis gamut.
Starting with understanding the difference between a proactive and reactive approach to PRCA, the benefits of each, and how to determine the triggers of an RCA investigation, we nailed down the RCA basics together.
Learning how to develop a timeline, identifying what information needs to be captured to support the analysis, and how to integrate RCA methodology with existing incident investigation processes followed.
With much to get through in just two days, we led our delegates through the ins and outs of impact statement writing, before launching into an analysis of the “5 Why’s” with some interactive exercises.
Getting into the nitty-gritty, analysis of cause and effect was next on the agenda to help set the foundations for delegates to identify solutions, before diving into the importance of evidence to support their investigations.
Next, the big one. The very purpose of RCA; solutions. Here delegates were guided through how to identify effective solutions to problems, before learning how to report their findings in a simple yet effective way,
To bring the second and final day to a close, delegates were split into small groups to complete a full investigation utilising the training received.
